Placements24 is seeking a highly skilled Remote Elderly Care Specialist to assess needs, create personalized care plans, and coordinate in-home and remote services for seniors. This fully remote role emphasizes compassionate, person-centered care and effective use of care management systems.

Applicants should have a relevant degree in Nursing, Social Work, Gerontology or allied fields, plus at least 5 years of elder care experience and proficiency with virtual platforms and EHRs.

Qualifications

• Minimum 5 years of experience working with the elderly in care coordination, case management, or home care settings.
• A relevant degree or diploma in Nursing, Social Work, Gerontology, Allied Health, or a related field.
• Strong knowledge of geriatric care principles and age-related health conditions.

Responsibilities

• Conduct remote assessments of clients' physical, cognitive, social and emotional needs.
• Develop and update individualized care plans.
• Coordinate services including in-home care, medical appointments and meal services.
• Liaise with families and healthcare providers to ensure seamless care delivery.
