Qualifications

• Relevant qualification in Social Work, Community Services, Psychology, or a related field.
• 3–5 years of experience in community support or social care, with at least 1 year in a senior/supervisory capacity.
• Experience developing and implementing support plans for individuals with diverse needs.
• Excellent communication, interpersonal, and active listening skills.
• Proficiency with digital communication tools and case management software for remote work.
• Strong organizational and time-management abilities.
• Ability to work independently and as part of a remote team.
• Compassionate, empathetic, and non-judgmental attitude.
• Willingness to undergo background checks and adhere to policies.
• Understanding of relevant legislation and best practices in community care.

Responsibilities

• Develop, implement, and review individualized support plans for clients, ensuring alignment with their needs and goals.
• Provide direct support and assistance to clients in various aspects of their daily lives, fostering independence and well-being.
• Conduct regular assessments of client progress and adjust support strategies as required.
• Liaise effectively with clients' families, external agencies, and healthcare professionals to ensure a coordinated approach to care.
• Offer emotional support, guidance, and advocacy for clients facing diverse challenges.
• Mentor and supervise junior community support staff, providing training and constructive feedback.
• Maintain accurate and confidential client records and documentation.
• Participate in team meetings, case conferences, and professional development activities.
• Manage caseloads efficiently and prioritize tasks to meet service demands.
• Uphold the highest standards of professional conduct and ethical practice at all times.
