Qualifications

• Master's degree in Social Work, Psychology, Counseling, or a related field.
• Minimum of 6 years of progressive experience in social work, case management, or community support.

Responsabilités

• Provide expert guidance to clients navigating social services, healthcare, and community resources via virtual channels.
• Conduct needs assessments remotely to understand client circumstances and required support.
• Develop personalized action plans and assist clients in accessing relevant services and programs.
• Act as primary point of contact and advocate for clients, ensuring rights and needs are met.
• Maintain detailed confidential client records using a secure digital platform.
• Collaborate with service providers and community organizations for referrals and transitions of care.
• Offer emotional support and guidance to clients facing challenging situations.
• Contribute to development of online resources and materials for clients and staff.
• Mentor junior support navigators on remote client engagement best practices.
• Stay updated on social policies and programs for remote support delivery.
