Job summary

Placements24 is seeking a dedicated Remote Case Manager to join a growing national team. This fully remote role focuses on coordinating care plans for individuals facing mental health challenges from anywhere in South Africa.

You will assess needs, connect clients to resources, and advocate for well-being via telehealth platforms. Ideal candidates hold a relevant degree, have SACSSP/HPCSA registration, and a minimum of 3 years in case management.

Qualifications

• 3+ years of experience in case management, preferably in mental health services.
• Ability to conduct comprehensive remote assessments and develop care plans.
• Proficiency with digital tools and maintaining confidential records.

Responsibilities

• Conduct comprehensive remote assessments of clients’ mental health, social, and functional needs.
• Develop, implement, and monitor individualized care plans with clients and professionals.
• Facilitate access to therapy, housing, employment support, and community resources.
