Qualifications

• Bachelor's degree in Social Work, Psychology, Disability Studies, or related field.
• Minimum 5 years of disability support, case management, or advocacy.
• Experience developing person-centered support plans.
• Strong advocacy and communication abilities.
• Ability to work independently and manage a caseload.
• Familiarity with Eastern Cape community needs.

Responsibilities

• Conduct needs assessments for individuals with disabilities in the Port Elizabeth region.
• Develop and implement individualized support plans focusing on independence.
• Provide direct support including life skills coaching and community integration.
• Advocate for clients' rights and access to services.
• Liaise with healthcare providers, government, educational institutions, and community orgs.
• Monitor progress and adjust plans as needed.
• Maintain confidential client records.
