Placements24 is seeking a dedicated Remote Disability Support Coordinator to empower individuals with disabilities in Stellenbosch, Western Cape. This fully remote role allows you to provide advocacy and coordinate services from your home office, developing personalized plans to foster independence and improve quality of life.

Qualificações

• Degree or diploma in Social Work, Occupational Therapy, Psychology, Disability Studies or related field.
• Minimum of 4 years in disability support, case management or related role.
• In-depth knowledge of disability legislation, services, and best practices in South Africa.
• Strong advocacy and commitment to empowering individuals with disabilities.
• Excellent remote communication, active listening, and problem-solving abilities.
• Proficiency with digital tools for remote work (video conferencing, CRM, electronic records).
• Ability to work independently, manage a caseload, and stay organized.
• Stable internet and a suitable home office for remote work.

Responsabilidades

• Conduct comprehensive needs assessments for clients with disabilities.
• Develop personalized support plans tailored to goals.
• Coordinate access to healthcare, therapy, education, and community programs.
• Advocate for clients and uphold their rights and needs.
• Build relationships with disability service providers and agencies in the Western Cape.
• Monitor and evaluate support plans, adjusting as needed.
• Support families and caregivers with guidance.
• Maintain accurate, confidential client records using secure online systems.
