Job summary

Placements24 is seeking an experienced Case Manager to spearhead community services initiatives in a remote-first capacity. You will assess, plan, and coordinate services for individuals and families facing complex social needs, leveraging technology to connect with clients across South Africa, with a focus on Rustenburg.

The role requires a Bachelor's in Social Work or Psychology, SACSSP registration, and at least 4 years of case management experience.

Qualifications

• Bachelor's degree in Social Work, Psychology, or related human services field.
• Professional registration with SACSSP or equivalent is essential.
• Minimum of 4 years of experience in case management or social services delivery.
• Proficiency in case management software and virtual communication platforms.
• Excellent organizational, time management, and problem-solving skills.

Responsibilities

• Conduct thorough client assessments remotely via video conferencing and phone calls.
• Develop, implement, and monitor personalized case management plans to meet client goals.
• Coordinate access to essential services including healthcare, housing, and employment support.
• Maintain detailed electronic case files with accuracy and confidentiality.
• Collaborate with a remote team of professionals, community partners, and service providers.
• Provide crisis intervention and support using digital communication tools.
