Job summary

Placements24 is seeking a highly organized Remote Case Manager to support clients across Stellenbosch and the wider Western Cape. You will assess needs, develop care plans, and coordinate services from your home, using digital tools to manage a caseload.

The role requires a Bachelor’s degree in a relevant field and at least three years in case management or community support, with strong communication and resource navigation skills.

Qualifications

Bachelor's degree in Social Work, Psychology, Community Development, or related field.
Minimum of 3 years in case management or community support roles.
Knowledge of social services, healthcare systems, and Western Cape resources.

Responsibilities

Conduct client needs assessments via video conferencing and phone calls.
Develop personalized case management plans outlining goals and required support services.
Coordinate access to healthcare, housing, and employment resources.
Maintain detailed electronic client records and progress notes.
Regularly communicate with clients, families and service providers to ensure continuity of care.
Monitor progress, address challenges, and advocate for client needs.
