Job summary

Placements24 is seeking a highly organized Remote Case Manager to support clients across Stellenbosch and the wider Western Cape. You will assess needs, develop care plans, and coordinate services from your home, using digital tools to manage a caseload.

The role requires a Bachelor’s degree in a relevant field and at least three years in case management or community support, with strong communication and resource navigation skills.

About the Role

This fully remote position allows you to provide essential support and guidance to clients from the comfort of your own home, focusing on the Stellenbosch and wider Western Cape region. You will assess client needs, develop comprehensive care plans, and coordinate services to help individuals achieve their personal and social goals. Key Responsibilities

• Conduct client needs assessments via video conferencing and phone calls.

Requirements

• Bachelor's degree in Social Work, Psychology, Community Development, or a related field.
