Key Responsibilities

• Provide remote assistance to clients seeking community and social care services.
• Conduct needs assessments via phone, video calls, and online platforms.

• Maintain accurate and up-to-date client records in a secure digital database.
• Follow up with clients to ensure services are received and needs are met effectively.

• Collaborate with internal teams and external partners to streamline service delivery.
• Troubleshoot and resolve client issues in a timely and empathetic manner.

Requirements

• A relevant qualification in Social Work, Psychology, Health Sciences, or a related field.
• Minimum of 2 years of experience in a client-facing role within social care, healthcare, or a similar service-oriented industry.
• Excellent communication skills, both verbal and written, with the ability to convey information clearly and concisely via digital channels.
• Proficiency in using video conferencing tools, CRM systems, and other relevant digital platforms.
• Strong organizational skills and the ability to manage multiple client cases simultaneously.
• Empathy, patience, and a genuine desire to help others.
• Ability to work independently with minimal supervision in a remote setting.
• Reliable high-speed internet connection and a dedicated home office space.
