• Strong knowledge of aged care, disability support, or home-based care.
• Ability to develop and implement care plans and supervise teams.
• Excellent communication, interpersonal and problem-solving skills.
• Valid driver’s license and access to reliable transport.

Aufgaben

• Supervise and manage a team of caregivers with guidance and performance feedback.
• Develop and implement client care plans in collaboration with clients, families and healthcare professionals.
• Conduct regular client assessments and reassessments to ensure care needs are met.
• Oversee caregiver scheduling and ensure adequate coverage and efficient resource allocation.
• Provide training and ongoing professional development for caregivers.
• Ensure adherence to care policies, procedures and regulatory requirements.
• Respond to client and caregiver concerns and resolve issues promptly.
• Conduct home visits for quality assurance and client satisfaction checks.
• Maintain accurate client records and caregiver documentation.
• Act as liaison between clients, families, caregivers and management.
