• Proven ability to lead and motivate a team of caregivers.
• Strong understanding of home-based care best practices and patient care standards.
• Excellent assessment, planning, and problem-solving skills.
• Exceptional interpersonal and communication skills, both written and verbal.
• Valid driver’s license and access to a vehicle for local travel in North West.
• Compassionate nature and strong commitment to client well-being.

Aufgaben

• Supervise, train, and mentor a team of home-based caregivers.
• Develop and implement individualized care plans for clients in collaboration with healthcare professionals.
• Conduct regular client assessments to evaluate care needs and satisfaction.
• Create and manage caregiver schedules, ensuring adequate coverage for all clients.
• Monitor caregiver performance and provide constructive feedback and support.

• Conduct in-home supervisory visits to ensure quality of care delivery.
