Qualifications

About the Role

Our client is seeking an experienced and compassionate Lead Care Navigator to join their fully remote team. This pivotal role involves guiding individuals and families through complex healthcare and social support systems. As a remote professional, you will be the primary point of contact for clients, offering personalized support, information, and resources to help them access the services they need. You will play a key role in developing and refining navigation strategies, mentoring other navigators, and ensuring the highest quality of client care. Key Responsibilities

• Assess client needs and develop personalized care navigation plans.
• Provide direct support and guidance to clients in accessing healthcare, social services, and community resources.
• Educate clients and their families about available services and eligibility criteria.
• Facilitate appointments and follow-ups to ensure continuity of care.
• Maintain detailed and confidential client records using our secure digital platform.
• Collaborate with healthcare providers, social workers, and community organizations.
• Mentor and train junior Care Navigators, providing feedback and support.
• Contribute to the development of new programs and protocols for care navigation.
• Track client progress and outcomes, reporting on key metrics.
• Serve as a liaison between clients and service providers.

Requirements

• Master's degree in Social Work, Public Health, Nursing, or a related human services field.
• A minimum of 7 years of experience in case management, patient advocacy, or care coordination.
• Proven experience in a leadership or mentoring role.
• In-depth knowledge of healthcare systems and social services relevant to Cape Town, Western Cape, ZA.
• Exceptional interpersonal and empathetic communication skills.
• Proficiency with virtual communication tools and electronic health record (EHR) systems.
