Job summary

Placements24 is seeking a Senior Community Support Worker in Midrand, Gauteng to lead and mentor a team of support workers while delivering person-centred care to clients with diverse needs, including disabilities.

The role blends direct client support with team leadership, requiring empathy, strong communication, and reliable record-keeping. A valid driver’s license and willingness to travel within the Midrand area are essential.

Qualifications

• NQF Level 4 in Community Services, Social Work, or related field.
• At least 5 years of experience in community services with disability focus.
• Experience in supervisory or team leadership role within social care.
• Strong understanding of person‑centred approaches and relevant legislation.
• Excellent communication, interpersonal and problem‑solving skills.

Responsibilities

• Provide direct, person-centred support to clients with daily living activities.
• Develop, implement and review individualised support plans with clients and families.
• Lead and mentor a team of Community Support Workers with training and feedback.
• Conduct regular team meetings to discuss progress and service improvements.
• Ensure care aligns with policies, procedures and legislation.
• Manage client case files with accurate, confidential records.
