Key Responsibilities

• Provide direct, person-centred support to clients, assisting with daily living activities, personal care, and community engagement.
• Develop, implement, and review individualised support plans in collaboration with clients, families, and relevant stakeholders.
• Lead and mentor a team of Community Support Workers, providing guidance, training, and performance feedback.
• Conduct regular team meetings to discuss client progress, case management, and operational improvements.
• Ensure all care is delivered in line with organisational policies, procedures, and relevant legislative requirements.
• Manage client case files, maintaining accurate and confidential records of support provided and progress made.
• Liaise with external agencies, healthcare professionals, and families to ensure holistic client care.
• Respond to and manage crisis situations effectively and empathetically.
• Contribute to the development and delivery of training programs for staff.

Requirements

• Minimum of NQF Level 4 in Community Services, Social Work, or a related field.
• At least 5 years of experience in community services, with a significant portion focused on supporting individuals with disabilities.
