Job summary

Placements24 is seeking a Senior Community Support Worker in Johannesburg to lead case management and provide comprehensive support for vulnerable adults. The role blends fieldwork with administrative duties in a hybrid setup, requiring strong advocacy and collaboration with families and services.

The ideal candidate will have a Social Work or related degree, 5+ years in community support, and experience mentoring junior staff.

Qualifications

• A relevant qualification in Social Work, Community Development, or related field.
• Minimum of 5 years of experience in community support, social care, or a similar role.
• Proven experience in case management and support plan development.
• Strong knowledge of social welfare legislation and best practices.
• Excellent communication and advocacy skills.
• Valid driver's license and willingness to travel within Johannesburg.
• Experience supervising or mentoring junior staff is highly desirable.

Responsibilities

• Conduct comprehensive needs assessments for vulnerable adults.
• Develop and implement personalized support plans in collaboration with clients.
• Provide direct support, including emotional, practical, and social assistance.
• Facilitate access to community resources, healthcare, and social services.
• Advocate on behalf of clients with relevant agencies and professionals.
• Supervise and mentor junior community support workers.
• Maintain accurate and confidential client records and documentation.
• Organize and lead community engagement activities and support groups.
• Liaise with families, caregivers, and other support networks.
• Respond to crisis situations with professionalism and de-escalation techniques.
• Participate in team meetings and contribute to service development.
• Conduct home visits and community outreach within the Johannesburg area.
