Qualifications

• A relevant degree or diploma in Social Work, Psychology, Community Development, or a related field.
• Minimum of 3-5 years of experience in a similar community support or social care role.
• Proven experience in case management, needs assessment, and support plan development.
• Excellent communication, interpersonal, and active listening skills.
• Strong understanding of the social challenges faced by vulnerable populations in South Africa.
• Ability to work independently and as part of a multidisciplinary team.
• Proficiency in data management and report writing.
• Valid driver's license and willingness to travel within the Gauteng province.
• Empathy, resilience, and a non-judgmental approach to client support.
• Knowledge of local community resources and support networks in Midrand is advantageous.

Responsibilities

• Develop, implement, and monitor individualized support plans for clients, addressing their specific needs and goals.
• Conduct regular assessments to evaluate client progress and adjust support strategies as necessary.
• Liaise with external agencies, healthcare providers, and educational institutions to facilitate access to services and resources.
• Provide emotional support, guidance, and crisis intervention to clients and their families.
• Organise and facilitate community outreach programs and workshops to promote awareness and engagement.
• Maintain accurate and confidential client records in compliance with organisational policies and data protection regulations.
• Collaborate with the wider support team to share insights, best practices, and collective problem-solving.
• Advocate for clients' rights and needs within the community and with service providers.
• Assist in the recruitment, training, and supervision of volunteers and support staff as required.
• Contribute to the development and refinement of support service protocols and frameworks.
