Job summary

Placements24 in Midrand, Gauteng, is seeking a Senior Community Support Worker to lead care for elderly clients in the community. You will conduct needs assessments, develop personalized care plans, and coordinate services with families and healthcare professionals.

The role requires a background in geriatric care, strong communication, and supervisory experience. A driver’s license and CPR/First Aid are advantageous as the organization emphasizes compassionate, compliant care.

Qualifications

• 5+ years of experience in community or aged care, with at least 2 years in a senior or supervisory role.
• Strong care planning, assessment and case management skills.
• Excellent empathy and interpersonal abilities to support elderly clients.
• Ability to document accurately and maintain confidentiality.

Responsibilities

• Conduct comprehensive needs assessments for elderly clients to develop personalized care plans.
• Provide direct support with daily living activities including personal care, mobility and meals.
• Administer medication as prescribed and monitor health status with timely reporting.
• Organize and facilitate social and recreational activities to promote engagement and wellbeing.
• Liaise with families, medical practitioners and community services for integrated care.
• Mentor and supervise junior staff, providing guidance and feedback.
• Maintain accurate and confidential client records and documentation.
• Advocate for clients' rights and needs within care systems.
