Placements24 is seeking a proactive Community Support Facilitator to join our team in Stellenbosch, Western Cape. You will identify community needs, help clients access social welfare, healthcare, education and employment opportunities.

You will build relationships with local service providers, organise awareness sessions, and develop personal action plans while maintaining confidential records. A diploma or degree in relevant fields and at least 3 years' experience are required; the role offers

Qualifications

• Diploma or Bachelor's degree in Social Work or related field.
• Minimum 3 years of experience in community work or case management.
• Strong knowledge of Western Cape social services.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.

Responsabilités

• Identify community needs and gaps in service provision.
• Facilitate access to social welfare programs, healthcare, education, and employment opportunities.
• Provide guidance and support to individuals and families navigating support systems.
