Job summary

Placements24 is seeking a proactive Community Support Facilitator to join our team in Stellenbosch, Western Cape. You will identify community needs, help clients access social welfare, healthcare, education and employment opportunities.

You will build relationships with local service providers, organise awareness sessions, and develop personal action plans while maintaining confidential records. A diploma or degree in relevant fields and at least 3 years' experience are required; the role offers

About the Role

Our client is seeking a proactive and empathetic Community Support Facilitator to join their team in Stellenbosch, Western Cape.

Key Responsibilities

• Identify community needs and gaps in service provision.
• Facilitate access to social welfare programs, healthcare, education, and employment opportunities.
• Provide guidance and support to individuals and families navigating various support systems.
• Develop and maintain relationships with local service providers and community organisations in Stellenbosch.
• Organise and conduct community awareness sessions on available resources.
• Assist clients in developing personal action plans to achieve their goals.
• Monitor client progress and the effectiveness of support interventions.
• Maintain accurate and confidential client records and submit regular reports.
• Advocate for the needs of community members with service providers.
• Collaborate with team members to enhance service delivery and program impact.

Requirements

• Diploma or Bachelor's degree in Social Work, Community Development, Sociology, or a related field.
• Minimum of 3 years of experience in community work, social support, or case management.
• Strong knowledge of social services and resources available in the Western Cape.
• Excellent communication, interpersonal, and problem-solving skills.
• Ability to work with diverse populations and build trust.
• Proficiency in relevant software for case management and reporting.
• Familiarity with the Stellenbosch community and its specific challenges.
• Ability to work both independently and collaboratively within a team.
• Valid driver's license and willingness to travel within the region is advantageous.
