Qualifications

• A degree in Nursing, Social Work, Child Development, or a related healthcare/social services field.
• Proven experience in pediatric care coordination, case management, or a similar role.
• Strong understanding of child development, common pediatric conditions, and treatment pathways.
• Exceptional communication and interpersonal skills, with the ability to build rapport remotely.
• Proficiency in using telehealth platforms, electronic health records (EHR), and other relevant software.
• Excellent organizational and time management skills, with the ability to manage multiple cases simultaneously.

Responsibilities

• Develop individualized care plans for pediatric patients in collaboration with healthcare providers and families.
• Coordinate appointments, therapies, and educational resources for children.
• Serve as a primary point of contact for parents and guardians, providing support and information.
• Liaise with doctors, specialists, therapists, and educational institutions to ensure seamless care delivery.
• Monitor children's progress and developmental milestones, adjusting care plans as needed.
• Maintain accurate and confidential patient records in compliance with privacy regulations.
• Identify and address potential barriers to care access and advocate for the child's needs.
• Conduct virtual check-ins and provide remote guidance and support to families.
