Job summary

Placements24 is seeking an experienced Remote Care Coordinator to join a fully remote team in South Africa. The role focuses on coordinating care for patients with chronic diseases, delivering education, and leveraging telehealth tools to improve outcomes nationwide.

The ideal candidate holds an EN or relevant degree with 3–5 years in chronic disease management, strong communication, and digital literacy to support remote patient engagement and care planning.

Qualifications

• Registered nurse or health/social care degree with current registration.
• Minimum 3–5 years in chronic disease management or community health settings.
• Experience with telehealth, EMR/EHR, and remote monitoring technologies.

Responsibilities

• Conduct remote patient assessments and develop individualized care plans for chronic diseases.
• Provide ongoing patient education on disease management and adherence.
• Monitor health status via telehealth tools and maintain documentation.
• Coordinate care with physicians and other providers.
• Assist patients in accessing community resources and support groups.
