Qualifications

About the Role

Our client is seeking an experienced and dedicated Case Manager specializing in the support of vulnerable adults for their team in Sandton, Gauteng . This hybrid role is pivotal in ensuring that adults facing complex challenges, such as disability, chronic illness, or social isolation, receive the comprehensive support and resources they need to live fulfilling and independent lives. You will be responsible for assessing individual needs, developing personalized care plans, coordinating services, and advocating for clients. The position requires a blend of in-person client visits and community engagement, alongside remote administrative and planning tasks.
