Job summary

The role requires a BSW and Social Work Certification, 2+ years of healthcare experience, and on-site work in the Waukesha, WI area with weekend shifts. Per diem and CMS/JCAO guidelines apply.

Qualifications

• Bachelor's degree in social work (BSW) required.
• Social Worker Certification required.
• 2+ years of experience in a healthcare setting.
• 2+ years of healthcare terminology knowledge.
• Ability to read, write, speak, and support patients and families; onsite in Waukesha WI.

Responsibilities

• Coordinate patient care processes to achieve quality outcomes and manage resources.
• Provide crisis intervention and psychosocial support; advocate and link to resources.
• Discharge planning and post-hospital service coordination with care team.
• Educate patients and families on options and levels of care.
• Maintain compliance with CMS and Joint Commission standards.
• Collaborate with hospital staff and community resources to promote patient wellness.
