You will support a collaborative clinical workflow, maintain patient privacy, participate in staff development, and ensure accurate documentation.

Qualifications

• Registered Nurse with current license (RN) and two years nursing experience.
• Current basic life support certification (CPR).
• Supervisory experience or aptitude.
• Strong computer skills preferred.
• Experience in community health or in a community agency.
• Ability to relate well to people from diverse ethnic and cultural backgrounds.
• Bilingual preferred (Spanish/English or Hmong/English).

Responsibilities

• Conduct wellness exams as assigned.
• Assist with triage and telephonic needs of patients.
• Assist in smooth workflow of clinic by supporting clinical floor activities and emergent/acute patients.
• Provide care coordination and develop treatment plans for complex patients.
• Ensure patient privacy and accurate medical record documentation.
