Job summary

Three Pillars seeks a Community Health Worker - Healthy Aging to support older adults and families across Waukesha, Jefferson, and Dodge counties. The role focuses on expanding access to cognitive health education, screening, care coordination, and supportive services through education, outreach, coaching, navigation, and advocacy.

As a grant-funded position, you will collaborate with healthcare providers, senior housing staff, and community partners.

Qualifications

• CHW certification or willingness to obtain within six months of hire.
• Minimum of two years of experience in community health, aging services, public health, or related field.
• Experience with dementia, cognitive impairment, aging populations.
• Strong interpersonal and communication skills.
• Knowledge of community resources and social service systems.
• Ability to work independently and within an interdisciplinary team.
• Proficiency with Microsoft Office and electronic documentation systems.
• Valid driver's license and ability to travel within the service region.

Responsibilities

• Provide ongoing support to individuals living with mild cognitive impairment, Alzheimer's disease or related dementias.
• Offer education, emotional support, and resource navigation for family caregivers.
• Build and maintain relationships with community partners.
• Facilitate educational presentations, workshops, staff training, and community events on brain health and dementia.
