Job summary

The Children’s Home of Wyoming Conference in Goshen, NY is seeking a Transition Coach for the Youth Empowerment and Support Transition Program. You will coordinate services for youth in foster care and alumni, work closely with the Program Supervisor and Case Worker, and help them develop concrete plans toward independence, housing stability, education, and employment.

• High School Diploma or GED required.
• Personally lived experience or work experience in child welfare required.
• Driver's license and ability to maintain insurability throughout employment required.

Responsibilities

• Coordinate services for youth in foster care and alumni; help develop independence plans.
• Assist youth with housing, education, employment, and community connectedness.
• Provide emotional and crisis support via in-person, phone, text and email.
• Assist youth with access to community resources and referrals.
• Attend court hearings and provide transportation as needed.
• Collaborate with youth, case workers, and program supervisor to meet needs and goals.
• Demonstrate leadership to motivate volunteers and interns.
