Job summary

Southwest Network seeks a Transition Facilitator to coach teens and young adults through the Transition to Independence Process. You will support employment, education, housing, and wellbeing as they move from child to adult services, in community settings with flexible hours.

You will work with partners across health, education, and welfare, developing individualized service plans and evaluating progress toward completion.

Qualifications

• Must have formal education in a behavioral health or related field.
• Experience with transition-aged youth in behavioral health or related systems is preferred.
• CPR, First Aid and Therapeutic Options certifications must be current.

Responsibilities

• Coach and support teens and young adults through transition to independence.
• Collaborate with child/adolescent, education, and adult services to plan progress.
• Complete administrative documentation and maintain records.

Skills

Behavioral health experience

Transition-aged youth experience

CPR and First Aid certifications

Education

Master's or bachelor's in behavioral health or related field

Bachelor's in unrelated field + 1 year related experience

Associate's + 1 year related experience

High school diploma + 3 years related experience
