Job summary

Wellroot Family Services seeks a Transition Support professional to provide direct help to youth, collaborate with Life Coaches, and promote safety and independence. The role emphasizes life-skills training, oversight, and community integration to empower clients toward self-sufficiency.

You will work closely with staff to implement individualized plans, document progress, and support youth across housing, budgeting, and daily living tasks in a structured program environment.

Qualifications

• Must be at least 21 years old and meet local regulatory requirements.
• Preferred degrees and advanced training in social services or behavioral health.
• Experience in Georgia child welfare or youth services is a plus.

Responsibilities

• Support youth in developing life skills and independence.
• Collaborate with Life Coaches to implement client plans.
• Provide supervision and safety oversight in daily activities.
• Assist with transportation to appointments and activities.
• Document progress and maintain confidential records.
• Deliver job search training and budgeting support.
