Job summary

Heartshare is seeking compassionate Coaches for the Evolve Youth Time Limited Rental Assistance Program in New York. You will guide at-risk youths toward rapid housing with private landlords, delivering coaching, advocacy, and service coordination in a strengths-based, trauma-informed manner.

Youth-focused roles require crisis intervention, collaboration with a multidisciplinary team, and ongoing documentation while supporting independence and independent living skills.

Qualifications

• Experience assisting with obtaining government benefits and accessing services.
• Ability to work with adolescent populations and groups.
• Capable of crisis management and maintaining documentation.
• Willingness to conduct home visits across New York.
• Strong written and verbal communication for clients and records.

Responsibilities

• Perform intake with youth following established guidelines.
• Complete needs assessments including mental health and social history.
• Assist with housing applications, employment and budgeting.
• Facilitate and participate in skill-building groups.
• Provide bi-weekly individual coaching and make referrals as needed.
• Maintain updated chart records and complete mandatory paperwork.
• Collaborate with staff to develop and evaluate programming.
• Conduct monthly CTI model home visits and attend staff meetings.
• Upload support plans and maintain caseload of 15+ tenants.
• Cultivate partnerships with community landlords and track documentation.
