Job summary

Children's Home is seeking a Transition Coach to coordinate services for youth aging out of foster care. You will work closely with a supervisor and case workers to help 16–21 year olds plan for independence, housing stability, education, and employment, building strong supports in their community.

Qualifications

• Must have a high school diploma or GED.
• Lived or worked in child welfare.
• Must possess a valid driver's license.

Responsibilities

• Coordinate services for foster youth and alumni to support independence.
• Build plans for housing stability, education, and employment.
• Provide ongoing emotional support via in-person, phone, text, and email.
• Connect youth with community resources and referrals.
• Assist with resume building, job applications, and college documents.
• Attend court hearings and transport clients as needed.
• Lead and motivate volunteers and interns.
• Help youths access vehicles and maintenance resources.
• Collaborate with youth, case workers, and supervisor to meet goals.
• Serve as a role model for program participants.
