Job summary

Open Sky Community Services in Massachusetts is seeking Transition Case Managers / Life Coaches to partner with youth and program clinicians to create and carry out a transition plan. You will work with young adults to promote independence, connect them with resources, and provide person-centered coaching focused on future goals.

Qualifications

• Bachelor’s Degree is required.
• Valid Driver’s License and acceptable driving record.
• Reliability, willingness to learn, and desire to promote a better living standard for youth and young adults living with mental illness or behavioral challenges.
• Availability to work occasional Saturdays based on program needs and scheduled events.
• Must have reliable transportation or consistent access to a vehicle to support outreach and other community-based responsibilities.

Responsibilities

• Outreach, including engaging young adults in the community, promoting program services, and connecting individuals with available resources.
• Follow and implement the standards and practices of the EBP Transition to Independence (TIP) Model.
• Engage young adults with one-on-one coaching that is person centered with future focused planning and goals.
• Provide strength-based, trauma-informed, and developmentally appropriate transition planning.
• Connect youth/young adult to services, entitlements, and more comprehensive levels of care as needed.
