Job summary
Wayside's Young Adult program in Somerville seeks a part-time Transition Facilitator to manage caseloads of 6-9 participants, assess needs, coordinate transportation, health and wellness tasks, budgeting and substance abuse supports. The role includes intake assessments, weekly case management, plan development, life skills training, employment assistance and strong community linkage.
Training will cover Motivational Interviewing and ARC, with supervision and policy development support.
Qualifications
- Bachelor's degree in counseling, psychology or related field.
- Experience in community-based or residential youth settings preferred.
- Valid driver's license and access to a reliable vehicle required.
- Bilingual/Bicultural candidates encouraged to apply.
Responsibilities
- Coordinate intake assessments for new participants.
- Schedule weekly case management meetings with assigned participants.
- Develop, implement, and evaluate service/participation plans with participants.
- Provide life skills assessment and education/training for participants.
- Provide employment assistance and apprenticeship job matching for participants.
- Develop and maintain a network of community-based support services.
