Job summary

Wellroot Family Services seeks a Transition Support worker to supervise youth, deliver life-skills training, and support independence in daily living. You will partner with Life Coaches to implement plans, track progress, and foster safety and growth in a community-based setting.

Responsibilities include budgeting support, job-search coaching, documentation, and coordination of transportation and medical appointments to empower youth toward self-sufficiency.

Qualifications

• High school diploma or equivalent required.
• BS in human services or related field preferred; MSW preferred.
• Two years of experience in a Georgia-regulated child welfare setting preferred.

Responsibilities

• Supervise and monitor youth progress per plans.
• Orient new clients to the program.
• Collaborate with Life Coaches to develop client plans.
• Teach life skills including budgeting, self-care, and daily tasks.
• Document weekly independent living activities per ISSP goals.
• Provide job search training and resume support.
• Assist with medical appointments and transportation.
• Maintain confidentiality and professional boundaries.
