Job summary

Intercept Health, part of the YouthQuest Independent Living Apartment Program in Virginia, seeks a Life Skills Coach/Case Manager to work with young adults aged 17–21 preparing for community transition. You will serve as a role model and help residents build money skills, gain employment, and pursue education.

Education options include a Bachelor's or Master’s in human services with relevant experience. The position offers strong benefits and a $2,000 sign-on bonus.

Qualifications

• Bachelor's degree in any field plus two years of case management experience with children and families.
• Or a human services Bachelor's with case management experience with children/youth and families preferred.
• Or a human services Master's with student placement in case management for children/youth and families.

Responsibilities

• Engage with young adults aged 17–21 to model positive values and behaviors.
• Support development of money management, employment, education, daily living skills, and career exploration.
• Contribute to living plans, life skills learning, transportation, and related administrative duties.
