Job summary

Southwest Network in Phoenix, AZ seeks a Transition Facilitator to coach and support teens and young adults as they move from child to adult services, using TIP principles. You will collaborate with schools and health providers to help youth access education, housing, employment, and health resources within flexible hours.

The role requires experience in behavioral health, strong communication, and the ability to navigate complex systems.

Qualifications

• Must possess one of the listed behavioral health–related degrees.
• Minimum one year of behavioral health experience preferred.
• CPR, First Aid, and Therapeutic Options certifications must be current.
• Arizona fingerprint clearance and driver's license required.

Responsibilities

• Coach and support teens and young adults to progress through the TIP program.
• Evaluate youth suitability and develop service plans with collaboration.
• Work in the community, including non-traditional hours, with other service providers.
• Handle administrative and documentation duties.
