Job summary

Gándara Mental Health Center in Chicopee, MA, seeks a Transition Coordinator | Case Manager for our Young Adult Therapeutic Care Program (ICS) to provide direct support to youth and families. You will act as Transition Facilitator to help young adults develop skills for independent living, coordinate services, and create housing, employment, and education plans with families.

The role involves working with clinicians, participating in meetings, and spending time visiting families and community

Responsibilities

• Provides services in a strengths-based, family-driven, youth-guided approach
• Coordinates referrals and placements for Young Adults in a timely manner
• Develops treatment and housing/employment/education plans with Young Adults
• Delivers in-home case management and safety planning for Youth and Families
• Teaches skills to enable independent functioning (e.g., transportation, budgeting)
