Open Sky Community Services seeks Transition Case Managers / Life Coaches to work with youth and young adults and a program clinician to create and carry out individualized transition plans. Responsibilities include outreach to engage participants, promoting services, and linking individuals to resources.

Training on the TIP model will be provided, with a focus on strength-based, trauma-informed planning and future-oriented goals.

Qualifikationen

• Bachelor's degree required.
• Valid Driver’s License and acceptable driving record.
• Reliability, willingness to learn, and desire to promote a better living standard for youth and young adults living with a mental illness or behavioral challenges.
