Transitional Residence for Youth in Topeka, KS is seeking a compassionate Case Coordinator to guide youth through critical transitions and connect them with resources. The role emphasizes holistic support, collaboration with partners, and empowering youth to build lifelong skills.

Responsibilities include developing individualized plans, coordinating services, and tracking progress with a trauma-informed approach to foster independence and success for each resident.

Qualifications

• Bachelor's degree in a relevant field.
• Experience in case management or youth services.
• Knowledge of trauma-informed care and youth development principles.
• Strong organizational and multitasking skills.
• Excellent written and verbal communication skills.
• Ability to work collaboratively with a multidisciplinary team and community partners.

Responsibilities

• Develop, implement, and monitor individualized case plans for youth residents.
• Coordinate services and referrals to community resources, including mental health, education, and employment support.
• Conduct regular check-ins and assessments to track client progress and adjust care plans as needed.
• Maintain accurate and up-to-date case documentation and client records in compliance with program standards.
• Collaborate with internal staff, external agencies, and family members to ensure holistic support for each youth.
• Facilitate life skills workshops and group sessions to promote independence and self-sufficiency.
• Advocate for youth needs within the organization and with outside service providers.
