Job summary

Community Solutions, Inc. seeks a dedicated Case Manager for the Youth MAP program in San Bernardino, CA.

You will handle intake, assessments, counseling, and case records, coordinating discharge and aftercare, with collaboration across sub-contractors at a single site. Qualifications include a Bachelor’s in social science and at least one year in case management, plus strong communication and Microsoft Office proficiency.

Qualifications

• Bachelor’s degree in a social science field and one year experience in case management or similar field.
• Excellent oral, written and interpersonal skills.
• Experience with Microsoft Office is preferred.

Responsibilities

• Conduct client orientation and intake data collection.
• Perform needs assessments and develop program plan goals with clients.
• Schedule, conduct and document individual counseling sessions and group meetings.
• Maintain client files and confidentiality per policy.
• Complete progress notes, discharge summaries and aftercare planning.
