Job summary

Community Solutions, Inc. (CSI) seeks a dedicated Case Manager for our Youth MAP site in San Bernardino, CA.

The role covers intake, assessments, counseling, case files, discharge planning and aftercare to support justice-involved clients in developing vocational and personal skills. The position requires a bachelor’s degree in a social science, at least one year in case work, strong written and oral communication, and proficiency with Microsoft Office.

Qualifications

• Bachelor’s degree in social science and one year experience in case management or similar field.
• Excellent oral, written and interpersonal skills desired.
• Experience with Microsoft Office.

Responsibilities

• Conduct client orientation, intake data collection and needs assessments.
• Develop and document program plan goals with clients and monitor progress.
• Provide individual and group counseling and assist with discharge planning.
• Maintain confidential client records and complete progress notes.
• Coordinate aftercare services and document outcomes.
• Assist in weekend scheduling, staff training and policy adherence.
