Job summary

Youth Opportunity is seeking a Case Manager to deliver case management services for youth in care under clinical supervision. You will coordinate treatment activities and facilitate communication between youth, families, probation, and external agencies.

The role supports reintegration planning, develops treatment and discharge summaries, and helps prepare families for visits and aftercare. Education required: Bachelor's degree in counseling, social work, psychology, rehabilitation, or related

Qualifications

• Bachelor’s degree in counseling, social work, psychology, rehabilitation, or related human services field is required.
• Two years of experience working with youth with emotional or substance use challenges is preferred.

Responsibilities

• Provide case management services to youth and families under clinical supervision.
• Coordinate treatment activities and facilitate communication between stakeholders.
• Develop and update treatment and discharge plans; support post-release tracking and outcomes.
• Serve as liaison between the program, DJJ, families, and external agencies.
• Prepare families for visitation, home leave, aftercare, and group/family meetings.
• Connect youth and families to community resources and ensure policy compliance.
