Job summary

Community Solutions Inc. in San Bernardino is seeking a dedicated Case Manager to support the Youth MAP program.

You will perform intake assessments, develop program plans, provide individual and group counseling, maintain case records, and coordinate discharge and aftercare. The role requires a Bachelor's degree in a social science field and at least one year of case management experience; proficiency with Microsoft Office and strong interpersonal skills are essential.

Qualifications

• Bachelor’s degree in a social science field and one year experience in case management or similar field.
• Excellent oral, written and interpersonal skills desired.
• Experience with Microsoft Office.

Responsibilities

• Conduct client orientation and collect intake data with needs assessments.
• Develop and document program goals; monitor treatment stipulations and job readiness.
• Develop discharge plans and schedule individual counseling sessions.
• Document group meetings, log activities, and maintain confidential client files.
• Provide services to Aftercare participants and coordinate with supervision.
