Job summary

NCS in the Bronx manages a supportive housing program for young adults aged 18-25. The Case Manager conducts intake assessments, develops treatment plans, maintains confidential files, and coordinates referrals to services while ensuring a safe, supportive environment.

The role requires a Bachelor’s degree (Masters preferred) in social work or psychology, experience with SMI/SED populations, and fluency in English; Spanish is a plus.

Qualifications

• Bachelor's degree in social work, psychology, or other acceptable equivalent.
• Demonstrated experience working with SMI/SED populations, preferably with young adults.
• Bilingual in Spanish preferred; strong English communication skills.

Responsibilities

• Conduct comprehensive intake assessments for new residents.
• Develop treatment plans within 30 days of arrival.
• Maintain confidential client files with accuracy.
• Monitor residents' medication intake and behavior.
• Provide referrals and coordinate events for tenants.
• Prepare monthly statistical reports and participate in team meetings.
