Job summary

CAMBA in Bronx, NY seeks a full-time Case Manager to support families through housing and social services programs. You will perform intake, assess needs, create and maintain client files, and connect clients with community resources to promote self-sufficiency.

Responsibilities include coordinating referrals, scheduling appointments, and documenting progress in CARES. You will advocate for clients and collaborate with diverse partners to ensure holistic support.

Qualifications

• Minimum education per the job posting with relevant experience.
• Proficiency in MS Office and basic office technology.
• Bilingual ability preferred (English plus another language).

Responsibilities

• Maintain professional relationships with clients and protect confidentiality.
• Input client data and progress into CARES database.
• Conduct initial intake and assess client needs; periodic re-assessments.
• Assist clients in attaining goals by identifying and locating resources.
• Refer clients to housing opportunities and services; schedule appointments.
• Escort clients to housing, entitlements, education, medical, and other appointments.
• Assist with benefit applications and processing; advocate for clients.
• Monitor progress and document outcomes; follow up with referrals as needed.
• Act as liaison with outside organizations for education, healthcare, housing, and legal matters.
• Prepare information for reports on a weekly/monthly/quarterly/annual basis.
