Job summary

Vibrant Emotional Health is seeking a Case Manager/Transition Facilitator in the Bronx Adolescent Skills Center. This on-site, full-time role supports young adults as they build independent living and life skills, using core practices to guide better decisions and improve outcomes.

Responsibilities include assessments, service plans, crisis intervention, coordinating health and mental health appointments, and documenting caseloads.

Qualifications

• Bachelor's degree in Social Work or a related field is required.
• Master's degree in Social Work, Mental Health Counseling, Psychology or a related field is a plus but not required.
• Minimum 1 year of experience providing case management to youth and families.
• Experience writing progress notes, managing case files, and providing counseling and conflict resolution.

Responsibilities

• Complete assessments, service plans, facilitate groups, provide crisis intervention and accompany youth to appointments.
• Assist youth with securing essential documents (SSN cards, working papers, IDs) and referrals to health and mental health services.
• Maintain accurate caseload entries and timely reports; supervise discussions and provide resolutions.
• Participate in meetings and trainings for professional development.
• Maintain professional conduct, time management and respectful culture.
• Provide counseling to youth and their families to foster support.
