Job summary

Bronx Family Network is seeking a full-time Case Manager to support residents in a DHS shelter in NYC. You will conduct needs assessments, develop ILPs, provide case management, crisis intervention, and connect clients with housing, health, and social services.

You will document in CARES, maintain accurate records, advocate for clients, and work closely with shelter staff to move clients toward permanent housing. Bilingual English/Spanish preferred; Microsoft Office proficiency required.

Qualifications

• Bachelor’s degree in social work, psychology, or human services.
• Minimum 2 years of case management experience.
• Experience with homeless or underserved populations and community resources.

Responsibilities

• Assess client needs and develop ILPs with ongoing case management.
• Conduct weekly/bi-weekly ILP reviews and document in CARES.
• Assist housing searches, job searches, and referrals to services.
• Maintain accurate documentation of intake forms and progress notes.
• Advocate for clients and connect with community resources.
• Ensure compliance with DHS/OTDA policies and shelter rules.
