Job summary

Vibrant Emotional Health in the Bronx, NY is hiring a Case Manager/Transition Facilitator to support young adults with social and emotional needs as they gain independent living skills. You will use core practices to help youths make better decisions and improve outcomes.

The role includes assessments, counseling, crisis intervention, records management, and coordination with health and developmental services. This is a full-time, on-site position with a strong benefits package.

Qualifications

• Bachelor's degree in Social Work, Psychology or related field.
• Master's degree in Social Work, Mental Health Counseling, Psychology or related field a plus.
• Minimum of 1 year of experience providing case management to youth and families.
• Experience writing progress notes, managing case files, counseling and conflict resolution.
• Proficiency in Microsoft Office and Google Suite.

Responsibilities

• Complete assessments, service plans, facilitate groups, provide crisis intervention, counseling and conflict resolution and accompany youth to health and mental health appointments.
• Assist youth with securing essential documents and connect to health, mental health and other support services.
• Maintain accurate caseload entries and other records; supervise and report incidents within 24 hours.
• Participate in supervision and professional development activities.
• Manage tasks with minimal supervision and maintain professionalism in dress, time management and culture.
