Job summary

Connected Community Schools seeks a Diversion Case Worker to provide individualized, community-based diversion services for at-risk youth to prevent foster care placement and juvenile justice involvement. The role emphasizes in-home visits, collaboration with providers, and comprehensive documentation.

The position is full-time (35 hours/week) with a flexible schedule, located in Utica. A bachelor’s degree in a related field and at least two years of relevant experience are required, with travel

Qualifications

  • Bachelor’s degree in social work, psychology, nursing or related human services field.
  • Two years of direct service experience with behavioral/mental health or juvenile justice populations (or a Master’s degree may substitute for one year).
  • Basic computer skills (Windows, Outlook, Word, Excel).
  • Travel required; valid NYS driver’s license.

Responsibilities

  • Provide diversion casework coordination and counseling to assigned cases.
  • Conduct at least two monthly contacts, including at least one in-home visit.
  • Collaborate with service providers and establish a team communication plan.
  • Maintain documentation in required HIT system and complete assessments and care plans.
  • Coordinate referrals and monitor services with families and youth.
