Job summary

The Village for Families and Children is seeking a Mid-Career Case Manager to provide comprehensive case management services in Hartford, CT. The role supports youth diversion and families, coordinating with the Police Department, schools, and community providers to ensure access to services and eligibility for programs.

Requires a BA/BS and 2–5 years of direct experience in behavioral health or SUD treatment; bilingual English/Spanish strongly preferred.

Qualifications

• Bachelor's degree required.
• At least 3 years of direct experience in SUD treatment.
• Bi-lingual English/Spanish strongly preferred.
• Culturally competent to serve diverse populations.

Responsibilities

• Engage clients from referral through treatment.
• Assist with eligibility for entitlement programs.
• Conduct standardized screening for health-related needs.
• Provide navigation to community services and Village resources.
• Maintain accurate program documentation and reporting.
• Collaborate with community agencies and referral sources.
• Adhere to HIPAA and funding requirements; protect client records.
